Hallazgos imagenológicos del vólvulo cecal, descripción por medio de un caso / Imaging findings of the cecal volvulus, description through a case

El vólvulo del ciego es la torsión intestinal que afecta al colon ascendente o al íleon terminal debido a un ciego móvil. Descrita por Bundschuh en 1913, representa el 1 % de los casos de obstrucción intestinal y del 10 % al 40 % de los vólvulos colónicos. Su incidencia es baja, afecta de 2,8 a 7,1 millones de individuos por año, con prevalencia en mujeres jóvenes. Se puede presentar en pacientes con antecedentes quirúrgicos, masas abdominales y estreñimiento. Las imágenes diagnósticas son importantes para su identificación: la radiografía de abdomen muestra hallazgos poco específicos, pero representativos; la tomografía computarizada muestra hallazgos más sensibles y específicos. A continuación, se presenta un caso clínico y por medio de este se revisan los hallazgos más importantes del vólvulo cecal.

The cecum volvulus is intestinal torsion that affects the ascending colon or terminal ileum due to a mobile cecum. Described by Bundschuh in 1913, it represents 1 % of cases of intestinal obstruction and between 10 % to 40 % of colonic volvulus cases. Its incidence is low, affecting 2.8 to 7.1 million individuals per year, with a higher prevalence in young women. It can occur in patients with a surgical history, abdominal masses and constipation. Diagnostic images are important for its identification; abdominal radiography shows nonspecific but representative findings, computed tomography shows more sensitive and specific findings. A clinical case is presented and the most important findings of the cecal volvulus are reviewed.

Vólvulo gástrico: aproximación diagnóstica mediante estudios especiales / Gastric Volvulus, Diagnostic Approach Through Special Studies

Introducción: El vólvulo gástrico es el giro del estómago sobre alguno de sus ejes. Objetivo: Realizar una aproximación diagnóstica mediante estudios especiales, describir los diferentes tipos y sus características. Materiales y métodos: Se realizó una búsqueda en el sistema PACS (por las iniciales en inglés de picture archiving and communication system) institucional (2017 a 2019), seleccionando los estudios más representativos con diagnóstico de vólvulo gástrico; posteriormente, se construyeron esquemas para facilitar la comprensión de los hallazgos. Resultados y conclusiones: Esta información le permitirá al radiólogo y, más aún, al residente, abordar este dilema clínico y reconocer por medio de esta excelente herramienta los hallazgos de los dos subtipos (organoaxial y mesenteroaxial), resaltando la importancia de nominarlo adecuadamente, considerando aquellos pacientes sin indicación quirúrgica de emergencia.

Background: Gastric volvulus is when the stomach turns on one of its axes. Objective: To make a diagnostic approach through special studies, describing the different types and their characteristics. Materials and methods: We conducted a search in the institutional PACS (Picture Archiving and Communication System) (2017 to 2019), selecting the most representative studies, with gastric volvulus diagnosis; subsequently, we built schemes to facilitate the understanding of the findings. Results and Conclusions: This information will allow the radiologist and, even more, the radiology resident, to address this clinical dilemma and recognize through this excellent tool the radiological findings of the two subtypes (organoaxial and mesenteroaxial), highlighting the importance of nominating the position, considering those patients with no emergency surgical indication.
